Team Information

This is the first point of contact for new referrals to Adult Social Care (excluding Hospital and Community Mental Health referrals). When you contact us, our first response team may arrange an assessment or signpost you to other services which could assist you as required. The focus of the team is the first assessment and safeguarding.We have a direct link with re-ablement.

The functions of the First Response Team are:

  • To receive referrals to determine eligibility for social care services and how urgent they are. The team will then complete an assessment.
  • To respond to and manage safeguarding referrals.
  • To identify if people have caring responsibilities themselves or if they have a carer and determine what support they may require.
  • To provide information, advice and guidance and signpost people as required.

If you are already receiving services from Adult Social Care the number you need to contact is and you will be put through to the appropriate team. Each of the Adult Social Care teams provides a daily ‘help desk’ service in office hours, where your query will be actioned by one of our officers.
